Settlement Class Members who wish to obtain compensation under this Settlement must submit a Claim Form. To be eligible for compensation, Claim Forms must be submitted on or before January 19, 2017.
To file a Claim Form online must have the Claim Number and Password sent to you on the Short Form Notice and Individual Summary. Consumers who believe that they are Settlement Class Members, as set forth in FAQ 5, but who have not received a Short Form Notice and Individual Summary, may contact the Claims Assistance Email Inbox at info@RhodeIslandTaxSettlement.com to request one.
NOTE: The tax charged for most Optional Service Contracts was between $2.00 and $30.00.
Some repeat Dell customers may have been assigned more than one Dell customer number and, therefore, will have been sent more than one Short Form Notice and Individual Summary.
To ensure full compensation under this Settlement, you must submit an Electronic Claim Form for each Short Form Notice and Individual Summary that has been sent to you. You may receive a separate payment for each qualifying Claim Form that you submit or such refunds may be combined into a single payment.
